DERMAL FILLERS: MINIMALLY INVASIVE TREATMENT FOR MAXIMUM RESULTS

Dermal hyaluronic fillers are, along with Botox, one of the most performed aesthetic treatments today. They have become widely used/popular in recent years.

They are popular because in a very short time, with a simple application, the desired correction can be achieved and make the look better and rested.

WHAT ARE DERMAL FILLERS?

Dermal fillers are gels based on hyaluronic acid that are applied to the skin and subcutaneous tissue to restore lost volume to your face, fill in wrinkles, emphasize cheekbones, lips or jawline and make your face smoother.

At the Lohuis Filipović Polyclinic, we use two groups of dermal fillers that differ in their structure, mechanism of action and longevity:

Dermal fillers based on hyaluronic acid

Hyaluronic dermal fillers are the most widespread group of fillers, and their basic role is to compensate for lost volume.

Hyaluronic acid is one of the main components of the space between the cells of our skin, i.e. the extracellular matrix, and after the age of 25, the processes of hyaluronic acid production in the skin persist and its amount begin to decline with age.

By applying dermal fillers based on hyaluronic acid, the face quickly and effectively restores the lost volume or additionally emphasizes the contours of the face.

In the clinic, we offer various hyaluronic dermal fillers, the effectiveness of which has been confirmed by numerous clinical studies. Due to various indications, but also the mode of action, we use hyaluronic fillers from renowned and well-known manufacturers, such as: Belotero, Neauvia, Teoxane, Juvederm, Restylane, Revanesse.

The structure of hyaluronic fillers is stable for approximately 6 to 18 months, which depends on the density of the filler and the place of application of the filler itself.

DERMAL FILLERS – COLLAGEN STIMULATORS

Dermal fillers, whose main role is to stimulate collagen production, are based on an ingredient called calcium hydroxyapatite or poly-L-lactic acid.

Calcium hydroxyapatite is a mineral compound similar to those found bones and teeth, making it a natural alternative for skin revitalization.

When injected into the skin, calcium hydroxyapatite helps stimulate collagen production, providing long-term anti-aging skin results. Its action is noticed almost immediately after injection, filling in wrinkles and restoring volume to the face.

In addition, we also use collagen stimulators based on poly-L-lactic acid – Sculptra and aesPlla, which are a popular alternative to hyaluronic fillers when we talk about restoring volume to the face. Sculptra, which has been present on the European market for more than 20 years, has already gained its justified status as a “must have” treatment for restoring skin firmness. While aesPlla has been described as the future of regenerative aesthetic medicine.

Poly-L-lactic acid is a biodegradable polymer of lactic acid that activates the natural processes of collagen production in the skin, and naturally restores the lost volume of the face.

The main difference between hyaluronic fillers and collagen stimulators is the longevity and mode of action. Collagen formation is a slow process, so collagen stimulators will gradually regenerate collagen stores in the skin weeks after application, so changes in the form of tightness and restored volume to the face are visible after 3 or more weeks from application, while volume replacement with classic hyaluronic fillers is noticed immediately after the treatment. Also, the result achieved with collagen stimulators lasts 24 months, in some patients even longer, unlike hyaluronic filler that retains its effect for up to a year.

In the clinic, we also have collagen stimulators from various manufacturers such as Radiesse, Sculptra, aesPlla.

It is not uncommon for us to often combine both types of fillers to achieve optimal results, depending on the area and the treatment goals themselves. In addition to the face, we can treat the neck, décolleté, hands.

WHAT IS LF BALANCE?

However, excessive use of hyaluronic dermal fillers can be counterproductive, creating an artificial and “puffy” look and causing unnecessary complications such as migration of fillers and a swollen appearance.

That is why we have developed the LF balance principle – a special and original approach of the Lohuis Filipović Polyclinic, which emphasizes a minimally invasive approach, with as few facial fillers as possible.

We are guided by the philosophy of “less is more”, correctly distributing minimal amounts of fillers on all defined points and layers of the face, creating visible, but natural results.

WHAT ARE THE GOALS OF DERMAL FILLER TREATMENTS?

Dermal fillers are suitable for anyone who notices the first signs of aging on the face, i.e. loss of volume, but also for those clients who want to emphasize the lips, cheekbones or jawline with dermal fillers. Therefore, the treatment with dermal fillers is recommended for everyone who has:

  • Thin lips, without contour and volume

Thin and unexpressed lips can create the illusion of a larger nose or a stronger chin. With fillers, we can even out such differences, filling the lips that will be proportional to the rest of the face.

  • Unexpressed facial contours

A smaller and weaker bone structure of the face can soften the contours that are considered attractive. With the right application of fillers, we can emphasize your facial lines.

  • Dark circles

Accentuated dark circles are one of the important factors by which our face loses its freshness and rested appearance. By applying fillers, we soften dark circles and give the face a fresher look.

  • Loss of volume in the face

Loss of volume in the face is a normal phenomenon that comes with age, and with dermal fillers we successfully fill in the resulting volume losses to achieve a youthful and revitalizing appearance.

  • Pronounced wrinkles and depression on the face

Deeper wrinkles, for which the application of Botox alone is not enough, can be filled with less dense dermal fillers or significantly alleviated with collagen stimulators.

  • Deep scars

Scars caused by surgeries, injuries or some other external factors can be successfully alleviated with dermal fillers.

  • Slight humps on the nose

Nasal asymmetries are a normal phenomenon and it is not uncommon for one side of the profile to be more prominent due to unevenness. With dermal fillers, we successfully even out these irregularities to achieve a symmetrical and straight nose.

WHAT DOES THE PREPARATION FOR DERMAL FILLER TREATMENT LOOK LIKE?

In order for the treatment to go smoothly and for the recovery period to be as fast as possible, it is necessary to properly prepare for the procedure.

Before the treatment, we make sure to conduct consultations where we will talk about your previous experiences such as Botox, laser therapies or some other aesthetic procedures on the face.

In addition to consultations, our team can carry out some of the following activities:

  • Analyses of your medical history and potential risks
  • Facial examination
  • Face photography
  • Suggest another option (if dermal fillers are not the best solution)

WHAT DOES DERMAL FILLER THERAPY LOOK LIKE?

Treatment of dermal fillers is carried out in several steps:

  1. Face assessment and mapping

Your plastic surgeon will examine your face, paying attention to texture and pigment. To make the procedure easier, the surgeon may mark certain points on the face as suitable sites for the injection of fillers.

  1. Facial cleansing and anesthesia

The selected places will be cleaned with an antibacterial solution. Cold compresses and anesthetic creams can be used to relieve pain and discomfort.

  1. Application of fillers

After disinfection and application of anesthesia, we apply fillers. After applying fillers, we massage the area and assess whether we have achieved the desired result, or whether additional amounts of fillers are needed.

Depending on the treated area, the treatment can last between 15-60 minutes.

  1. Facial cleansing and recovery

After satisfactory results, we cleanse the face with a special cosmetic agent, and in case of greater sensitivity of the face, we can apply specialized makeup that will protect your face from external factors.

WHAT DOES RECOVERY LOOK LIKE AFTER TREATMENT?

After the treatment, you will be given cold compresses that you will keep on your face to relieve possible discomfort and pain.

You can return to your daily activities immediately after the procedure.

In some cases, satisfactory results are achieved with just one treatment.

Depending on the choice of filler, it is necessary to choose the correct time interval in which you will repeat the procedure. For hyaluronic acid-based fillers, it can be as early as one year, while fillers based on calcium hydroxyapatite or poly-L-lactic acid will work for up to 4 years.

WHAT ARE THE POSSIBLE RISKS OF DERMAL FILLERS?

Before carrying out the treatment, we will introduce you to the possible risks of applying dermal fillers. Although our clients associate only positive experiences with dermal filler treatments, the risks of such treatment should not be ignored.

The most common and in most cases normal side effect involves mild swelling that usually disappears after a few days.

Of the more serious side effects, the following are possible:

  • Allergic reaction

An allergic reaction can occur due to the use of anesthesia. Possible allergic reactions to anesthesia should be discussed with a doctor.

  • Asymmetry of the face

The asymmetry of the face may be visible only a few days after the procedure, when the swelling disappears. Facial asymmetry in most cases is due to poor judgment and careless application. The resulting asymmetry is easily corrected by the subsequent application of fillers.

  • Long-term soreness and discomfort

If the needle hit the nerves on the surface of the face, there is a chance that you will feel pain to the touch 1-2 weeks after the treatment has been carried out.

  • Infections

Improper disinfection, as well as poor home care, can cause bacterial infections on the skin. Such complications are usually treated with antibiotics.

  • Bumps

Lumps can occur if an inadequate filler is applied to an inadequate layer of the skin. Or if the patient has not followed the instructions on home care after treatment.

  • Vascular occlusion

Although very rare, it is possible that the filler is applied to the artery or the pressure of the filler itself on the artery, which can lead to hypoxia and tissue necrosis. Cases of blindness have also been described in the literature. That is why it is extremely important that the filler is applied by an experienced doctor who knows the anatomy of the face and the technique of application, and who will know how to react in a timely manner even in the case of the rarest side effects so that the consequences of them are as mild as possible.
